In section 0.1, "Enterprise Side", you note "ISSUE: Examples in [1] show
user=phone. Why?"

The answer is that when the user-part of the URI is intended to specify
a PSTN number, RFC 3261, section 19.1.1, item "URI parameters", subitem
"user parameter" says that when the user-part is intended to specify a
PSTN number using the format of a "telephone-subscriber" (as defined in
section 25.1, and in RFC 2806), it is preferred to apply the
"user=phone" URI parameter to ensure that the user-part is not mistaken
for a user name from some other namespace.

Also, in "Service Provider Side", there's a typo:

   From: “Anonymous” <sip:anonymous at serivep.com>;tag=zd345

should be 

   From: “Anonymous” <sip:anonymous at servicep.com>;tag=zd345
